REGISTRATION FORMS (TO REGISTER WITH THE PRACTICE)

Patients can register with our practice by clicking on both links below.  You should complete both forms fully and  hand them into the practice.  A receptionist will go over the forms with you when you attend to ensure they have been completed correctly.  If you require assistance whilst completing the forms, please contact the practice on 01382 543251 and a member of our reception team will be happy to assist you further.  You will also find a copy of our practice information leaflet below which provides information regarding the practice and the services provided.

PLEASE NOTE - BOTH THE PATIENT REGISTRATION FORM AND PATIENT QUESTIONNAIRE MUST BE COMPLETED BEFORE HANDING INTO THE PRACTICE.

ONLINE (APPOINTMENTS / PRESCRIPTION) REGISTRATION FORM

Our online services allow patients to book or cancel appointments or request repeat prescription medication.  By using our online services, you can save time when seeking to arrange or cancel an appointment given the practice telephone lines can become extremely busy.

Patients can register for our online services by clicking on the link below, reading and completing the form fully and either handing into the practice or emailing to us using the email address listed on the form.

PHYSIOTHERAPY SERVICES

Patients can self refer for physiotherapy services by calling the Musculoskeletal Advice and Triage Service on 0800 9719390.  This service is designed to assist you should you need help with a muscle or joint problem.

Patients can also self refer to the Pelvic Health Physiotherapy service by clicking on the link below, completing the form and posting to the the address on the form.

PODIATRY SERVICES

Patients can refer themselves to NHS Fife Podiatry Service by clicking on the link below.  Podiatrists assess and treat problems of the foot and lower limb.  You can self refer to the podiatry service for advice without seeing your GP or other healthcare professional.  Please note, however, that the NHS Fife Podiatry Service does not provide a nail cutting service.  Completed forms should be emailed or posted to the address detailed on the form.

SELF CERTIFICATE FOR FIRST 7 DAYS OF ILLNESS

If you are absent from work due to illness, you should self-certify for the first 7 days.  Please click on the link below which will take you to HMRC website where a self-certificate can be downloaded and completed.
