To save patients time, the practice now accepts any request for an extension to a sick line (MED3) via email. Any request will be acknowledged by the practice as having been received and will be available to collect from the practice after 2 working days. If the doctor refuses an extension to your sick line, the practice will contact you to advise further.
When emailing your request through to the practice please detail:
FULL NAME
ADDRESS
DATE OF BIRTH
REASON FOR SICK LINE
HOW LONG YOU WISH THE EXTENSION TO BE FOR AND FROM WHAT DATE
WHERE YOU WISH TO COLLECT YOUR MED3 FROM (NEWPORT / TAYPORT)
You should email your request to Fife.F21609Tayview@nhs.scot and place "SICK LINE REQUEST" into the subject box.
Please note, collection from Tayport is only available on Tuesday and Thursday mornings.
